Founded in 2007, the Hong Kong Pharmaceutical Care Foundation is a registered non-profit charitable organization.
The foundation was founded by pharmacists who wish to provide pharmaceutical service and improve elderly medication use in old-age homes by using their expertise and professional knowledge. Major service includes visiting pharmacist, electronic medication administration record, automatic medication packaging service, electronic drug administration system and drug management training.

Aging population is a worldwide trend, with many elderly suffering from various chronic diseases. More medications are used to treat these chronic diseases. Physiological changes in the elderly, resulting in functional decline, coupled with cognitive decline. Therefore, it is common that elderly are unable to read their drug label correctly, differentiate the colour and shape of their medications. This makes elderly a high risk group for medication incidents. The medical cost for elderly is much higher than patients from other age groups. With limited public health resources, elderly medication use should received more attention.
Promote general awareness and knowledge about the correct and safe use of medications
Through a series of educational outreach events, including drug talks and roving drug knowledge exhibitions,we help enhance the overall awareness and understanding of proper and safe use of drugs in elderly. Free pharmacist counseling service is also provided for elderly to learn how to optimise their medication use and drug storage.
To provide professional pharmaceutical care services for old-age homes
To establish and implement safe and excellent drug management systems; to strengthen the connections between the profession and the public health and social welfare sectors; to cooperate with the Government and other relevant bodies, to achieve the above-stated objectives.
