Application criteria
1. Valid HKID card holder
2. Adult patients of any public hospitals in Hong Kong
3. Meet the medical conditions specified by this program
4. Having the original prescription of Lynparza™ issued by any Hong Kong public hospital
5. Having the purchase invoices and receipts of Lynparza™ issued by any Hong Kong public hospital
If applicant has already enrolled in any other Lynparza™ drug assistance program, applicant can only apply this program with prior approval only
